Mental Health

The North Durham Family Health Team (NDFHT) provides brief therapy to patients of Medical Associates of Port Perry (MAPP) and the broader North Durham community.  To provide equitable access to services, individual therapy appointments are reserved for those who are patients of MAPP and who do not have access to health care benefits or an Employee Assistance Program (EAP) through their employer. Evidence-based Cognitive Behavioural Therapy (CBT) Group Programming for Anxiety/Depression or Insomnia is available to all individuals living in North Durham Region age 18 years and older. Therapists at the NDFHT hold a Master of Social Work and are Registered with their respective College.

If you are interested in booking an appointment with our mental health team to discuss community resources, group programming or individual therapy, please book an intake appointment by clicking the link below or contacting 905-985-2895 x6070: 

The Intake Appointment: 

  • This appointment will be over the phone with one of our Social Workers and takes approximately 30 minutes. 
     
  • The purpose of this appointment is to gather more information about your current situation/symptoms and what kind of support you are looking for. 
     
  • During this appointment you may be directed to community resources that are the most appropriate for the support you are looking for. 
     
  • We will assess if it is most appropriate for you to engage in individual therapy or group therapy through the North Durham Family Health Team or access resources in the community. 
  • DO YOU HAVE EXTENDED HEALTH BENEFITS? If you have coverage for counselling, we will direct you towards private therapists in your area that you may contact. We encourage you to seek these services to reserve North Durham Family Health Team services for those who do not have coverage or cannot afford services. 

Please note mental health programming through the North Durham Family Health Team is intended for patients with mild to moderate mental health concerns. If you are experiencing a mental health crisis, please contact:
