Memory Clinic

The purpose of the Memory Clinic program is screening, diagnosis, and management of cognitive changes in the aging population. The program was developed to enhance the patient experience, and to improve health care outcomes in our community.  The Memory Clinic program is facilitated by Physicians, Nurses, Nurse Practitioners, Pharmacists, and Social Workers.

NDFHT Events

The NDFHT Memory Clinic is a program for patients aged 65 and over who are experiencing changes to their memory. Our team is made up of doctors, nurse practitioners, nurses, social workers and pharmacists. Together the team performs a full assessment for the causes of memory changes and recommends changes to improve memory function.

Our target population includes the following:

    • Adult patients with memory concerns/cognitive changes needing assessment and management suggestions
    • Adult patients with cognitive changes for driving assessments
    • Family concerns of a patient with cognitive changes in need of assessment
    • Caregiver supports for those caring for a patient with cognitive impairments in the home
    • Patients with change in ability to perform activities of daily living secondary to cognitive changes
