Chronic Pain Program

The North Durham Family Health Team Chronic Pain Program aims to improve pain management and safe/appropriate use of opioid medications.

Chronic Pain Program

The Pain program offers an initial one-on-one consultation with the Pharmacist, and follow ups every 1 to 6 months thereafter with the goal of improving pain management and safe/appropriate use of opioid medications. Referrals will be made for meditation therapy, mental health support, smoking cessation, chronic pain workshops, dietitian and social support.

The Pain program is facilitated by Pharmacists
