Social Worker

Part-Time

The Social Worker will be responsible for working with physicians and other IHPs (Integrated Health Professional) to assist individuals, families and groups in dealing with complex social issues that may be affecting their overall health. The Social Worker will provide counselling and/or therapy to patients and their families to assist them in managing their overall health.

Duties and Responsibilities:

Patient Assessment and Therapy

  • Provide needs assessment of patients to establish, implement and coordinate appropriate treatment plans
  • In coordination with physicians and IHPs, implement and discuss appropriate care plans for patients and the families
  • Facilitate coordinated access to support services such as assistance with daily living, crisis intervention, addiction, treatment health promotion and prevention
  • Work with community agencies, schools, Public Health, to develop a coordinated approach to the delivery of mental health services in the community
  • Facilitate coordinated access to various support services
  • Provide mobile mental health services in the communities of North Durham

Education

  • Develop and deliver mental health education programs for patients and their families
  • In collaboration with other IHPs, and based on the needs of the patients, [1]design, develop and deliver group and individual education programs related to chronic diseases such as diabetes, obesity, asthma etc.
  • Develop and update educational material for individual and group sessions and distribution to other NDFHT health professionals

Administration

  • Prepare monthly or statistical reports as required by the NDFHT
  • Actively participate in interdisciplinary, staff, team and committee meetings
  • Participate in NDFHT educational programs and relevant external workshops, courses and conferences
  • Contribute to and participate in overall NDFHT planning, programming and evolution

Qualifications and Relevant Experience:

  • Completion of a Master’s degree in Social Work, Psychology or equivalent
  • Current member of the Ontario College of Social Workers and Social Service Workers or College of Registered Psychotherapists of Ontario
  • Three to five years of experience as a Social Worker
  • Experience with group facilitation and individual psychotherapy
  • knowledge and experience working from a Cognitive Behavioural Therapy or Narrative approach
  • Experience with couples and family therapy would be an asset
  • Experience with multidisciplinary teams and community collaboration
  • Professional liability insurance
  • Knowledge of chronic disease management and relationship of mental health to various health conditions
  • A valid driver’s license and access to a vehicle necessary for home visits

Skills, Knowledge Requirements:

  • Previous experience in program development and evaluation
  • Proven ability to work in and contribute to an inter-disciplinary health care environment
  • Exceptional oral and written communication skills
  • Ability to work independently with minimal supervision
  • Ability to prioritize tasks, manage time and multi-task effectively in a very fast-paced work environment
  • High degree of computer proficiency
