Diabetes-education-icon Health Care Navigator

The Congestive Heart Failure (CHF) clinic offers an initial one-on-one consultation with a Pharmacist and a Nurse Practitioner, and regular follow ups thereafter.  The CHF clinic is a multidisciplinary program to manage heart failure patients with a reduced ejection fraction (EF < 40%). The goal of the clinic is to optimize pharmacotherapy, improve exercise tolerance, monitor fluid/electrolyte balance, while providing education on diet and lifestyle.  The team includes a nurse practitioner, pharmacist, dietitian and a cardiologist.  Additional referrals of smoking cessation, cardiac rehab and ICD assessments are performed as required.
