About
Recently, the North Durham Family Health Team was awarded expansion funding to ensure that unattached clients living in North Durham have access to timely, high-quality primary health care. Unattached is defined as someone who does not have a primary care provider (family physician or nurse practitioner). With this expansion funding, the North Durham Family Health Team has hired additional Nurse Practitioners to increase access to primary health care services for unattached residents living in North Durham.
Although this additional funding is only guaranteed until the end of March of 2025, we are optimistic that our funding will continue. With that in mind, while you are attached to a Nurse Practitioner at the North Durham Family Health Team, you are welcome to keep your name on the waiting list for a physician at Medical Associates of Port Perry. You may transfer your care to a physician at Medical Associates at any time.
We are committed to providing you and your family with accessible and timely care.
Primary Health Care Nurse Practitioners are fully qualified primary care providers, including assessing, diagnosing and managing acute and chronic conditions. This includes ordering diagnostic tests, prescribing medications, and sending referrals when necessary.

To register to attach to a Nurse Practitioner with the North Durham Family Health Team, please complete this online form.
A nurse will contact you as space permits to begin the intake process. Due to our funding allowance, space is limited so we cannot accept all applications.
