About Us

The North Durham Family Health Team (NDFHT) is an Ontario Health funded service to assist clients obtain their best health. 

We are a team of skilled allied health professionals working collaboratively to serve and expand medical services within Port Perry and its surrounding community.  Our team is comprised of the following professionals:

Formed in 2011 as part of a Ministry of Health initiative, our priority is to help people achieve their best health. We are committed to ensuring patients have access to timely, high-quality medical services close to home. To help people achieve their best health, the NDFHT offers many programs and services at no cost to the patient.

Many of our programs and services are available in person and virtually and we are located within Port Perry and its surrounding community. Our team collaborates with health care partners in the community, working together to avoid duplication, while providing seamless and secure communication to ensure patients have access to timely, high-quality primary care.

We focus on inter-professional programs focused on chronic disease management, disease prevention, and health promotion, and we fully support a care delivery system designed to improve patient and population health outcomes. By holistically designing care for our clients, we intend to improve client and population health outcomes; client, family and caregiver experiences; provider experiences; and value.

Message from Executive Director

It has been one year since embarking on my journey as Executive Director of North Durham Family Health Team (NDFHT). As I reflect on the past year, I am thrilled to share some exciting developments and achievements within NDFHT. Our commitment to providing exceptional healthcare services has led to significant expansion, including the establishment of a new Nurse Practitioner Clinic and Unattached Clinics in Brock and Port Perry. In addition, we have expanded our Children’s Clinic, Memory Clinic, and spirometry services. These additions allow us to offer more comprehensive and accessible care to our community, ensuring that everyone receives the healthcare they deserve.

As a primary health care team, our collective goal remains steadfast: to achieve 100% attachment for all individuals living in the North Durham region. We believe that every person should have access to a dedicated healthcare provider, and we are working tirelessly to make this a reality. This goal is not just a target but a promise to our community, reflecting our dedication to improving health outcomes for all.

We are positioning ourselves at the forefront of innovative healthcare solutions, ensuring that our community benefits from the best possible care. Over the past year, we redeveloped our website, expanded our use of digital tools, and leveraged technology to provide connected and convenient care.
